Reaction Questions

1.    From your perspective, why is it important to have community service providers at the table for transition planning? 

  • What do the agencies bring to the table?

2.    In your opinion, what might be the risks and benefits in involving community service providers in transition issues prior to the student exiting school?

3.    Currently, how are community service agencies involved with the schools and the IEP process for transition planning?  

  • Who invites the stakeholders together?
  • Do they meet before, during, or after the transition IEP team meeting?
  • What are some examples of that interaction?

Application Questions

1.    Are you aware of what agencies are service providers for adults with disabilities in your community? 

  • If so, who are the agencies we need to partner with to support transition?
  • If not, how can we go about finding out what agencies exist and what services they provide?

2.    What resources/technology can be developed to facilitate team members to quickly and efficiently gain access to appropriate community/resources/agencies? 

3.    When is it appropriate to invite stakeholders to participate in an IEP team meeting?

  • What factors should we take into consideration in order to determine when/how early to pull in these resources/agencies (i.e. at what grade level should they become involved to provide input, resources, etc.) to adequately support the youth and family?

4.    What new knowledge is required by community service providers to implement the transition to post school outcomes provision of IDEA?

5.    How can we create and sustain an environment for creative problem solving amongst partners, focusing on the student and his/her transition needs/services?
